Position Summary

Training is virtual.

Top 3 skills would be knowledge of basic medical terminology, effective communication and strong customer service skills.

Care Management Associate provides care coordination to facilitate delivery of appropriate quality healthcare, promote cost effective outcomes and improve program/operational efficiency involving transition of care and member engagement. Interact with members to deliver excellent customer service. Promotes and supports quality administration of care coordination. Works closely with Care Management for member referrals needing a higher level of care. Identifies members as appropriate for Health Risk Assessment (HRA) and Post Discharge Questionnaire (PDQ) upon discharge.

Duties

Outreaching members to complete Health Risk Assessment (HRA). Entering mailed in HRAs and following up on any care coordination needs identified. Outreaching members within 3 days of hospital discharge to complete a Post Discharge Questionnaire (PDQ) to prevent hospital readmissions. Reviewing Low Risk Care Plan goals with members and referring to PCP and/or CM for any unmet goals. Obtaining a Member Centered Goal as per new CMS guideline.

Experience

1+ years of experience in a healthcare setting.

1+ years of experience with basic medical terminology and concepts used in care management.

Computer literacy in order to navigate through internal/external computer systems, including Excel and Microsoft Word.

Effective communication, telephonic and organization skills.

Strong customer service skills to coordinate service delivery including attention to customers, sensitivity to issues, proactive identification and resolution of issues to promote positive outcomes for members.

Ability to effectively participate in a multi-disciplinary team including internal and external participants.

Education

High school diploma or GED required

Notes :

Monday-Friday 8am-5pm local AZ time; training is for 6-8 weeks
