Position Overview



Position Overview:
The pharmacist will be responsible for medication therapy management (MTM) services, including outbound calls for comprehensive medication reviews (CMR) and delivering in-depth consultations on statin medication use per AHA guidelines. The pharmacist will transcribe patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application. The data to be transcribed will be either hard-copy or available from client application. The data entered by the pharmacist will be used to create a standardized document that will be mailed to individual patients. The pharmacist is a member of the MTM product team.

Qualifications:

Education/Licensing Requirements:
Valid Pharmacist License in FL

Experience Requirements
At least 4 years of experience as clinical MTM pharmacist. (Management experience preferred)
Familiar with driving Part D Star measures. Specifically, SUPD, SPC, and CMR.
Understanding of pharmaceutical and medical vocabulary
Clinical experience in medication review.

Essential Skills/Experience:
Fluently speak, read, and write English
Excellent verbal and written communication skills
Experience using salesforce preferred
Must be tech savvy

Detail-oriented
Sense of urgency

Essential Job Responsibilities:
Conducting outbound phone calls. (100 calls per day)
Completing comprehensive medication reviews.
Conducting medication therapy management services.
Accuracy and speed in entry of patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application

Additional Job Responsibilities:
Attend training sessions to acquire/enhance skills related to Essential Job Responsibility
