Job Description

The position provides support to the Medicaid Care Coordination team by outreaching to Medicaid members to complete their initial health risk screening and completing door knocks when necessary to contact hard to reach members.

JOB REQUIREMENTS:
Bachelor Degree OR 4 years’ health care, social services or community advocacy experience,
Knowledge of medical terminology
Experience coordinating member related needs, providing assistance to members, and analyzing member needs
PC proficiency including Microsoft Office applications
Customer service skills

Preferred requirements
Community Health Worker (CHW) certification from the client approved college
Bachelor’s degree in Social Work

Notes:
Telephonic, Work from home position, in case they need to go onsite for a meeting (seldom)
Starting shift 9:30-6pm Monday - Friday
Once full business hours of 9am-8pm are open shift would be 12-8 Monday – Thursday and 9:30-6 on Fridays
