Description: The position provides support to the Medicaid Care Coordination team by outreaching to Medicaid members to verify that waiver services have started timely and to assist with trouble shooting if they have not. The position is also responsible for outreaching members to complete health risk screenings and providing coverage for any additional outreach efforts that are needed.
