Description:Manage Unable to Reach Population. Staff conducts telephonic and home visits with members to obtain a Health Risk Screening or Health Risk Assessment. Staff are expected to make 10-15 home visits per day.Staff are expected to travel within 50 miles of their home residence. Staff must have a reliable vehicle and vehicle insurance.This position is responsible for contacting members and community partners in assigned areas and is responsible for performing ongoing functions to support the Social Service department.Responsible for contacting identified members to inform and educate them on health care programs, responding to inquiries from members, and supporting the clinicians in the Physical and Behavioral Health Management department with their provider and member activities.
