Description:Position SummaryNurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will report to the practice manager.DutiesProvide Primary and Family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.Provide administrative duties to include but not limited to opening/closing the clinic, inventory, patient follow up, verifying insurance and collecting payment, maintaining a clean environment.Be responsible for handling patient appointments and performing routine examinations to ensure positive patient health outcomesPerform duties including speaking with patients about healthy lifestyle choices, completing diagnostic tests to help diagnose and treat patients for injuries or illnesses and coordinating with other healthcare professionals to maintain a comprehensive plan of careEncourage patients to become active participants in their own healthcare and well-being through services such as health screenings, behavioral health and wellness, chronic condition managementMaster's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse roleA minimum of 1 year of recent Primary Care experience as a Nurse Practitioner/Physician Assistant in either a family medicine or primary care practice preferred. If not, successful completion of the client primary care training program.Working knowledge and understanding of quality measurement in the management of chronic disease conditionsSelf- motivated, prioritizes and solves problems, takes initiative, and advocates for their patients and their practiceEffective verbal, written, and electronic communication skillsOutstanding organizational skills and ability to multi-taskInitiative, problem solving ability, adaptability, and flexibilityAbility to work remotely in a clinical care team cultureAbility to work without direct supervision and practice autonomouslyIs proficient with information management and technologyWillingness to obtain multi-state licensuresCapacity to collaborate with professional colleagues frequently to ensure quality patient careEducationMaster's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role.Notes:40 hours.
