Description:Physical TherapySetting:Primarily outpatient.Inpatient acute/swing-bed rehab coverage on a less frequent basis (estimated less than 5 hours/week).Patient Population & Case Types:Outpatient:o General orthopedics and practice PT (highest case mix includes shoulder, back, hip, and knee diagnoses).o Falls/balance referrals.o Neurological diagnoses (e.g., stroke, Parkinson’s).Inpatient:o Critical-access hospital with a small swing-bed rehab unit (approximately 15 patients).o Diagnoses include orthopedic injuries, post-surgical recovery, and general illness rehabilitation post-acute stay. Specialization Areas:Not required, but therapists on-site specialize in vestibular rehab.High geriatric case mix (>50%).Case Volume:Total of 15 sessions/day (30 minutes each).Evaluations: 1 hour each (limited to 4-5 per day).Follow-ups: 30 minutes each.No double/over-booking in usual practice.Technology & Modalities:Modalities: Ultrasound & e-stim.EMR: Epic for both inpatient and outpatient settings.Clinic features "computers on wheels" for real-time documentation.Certifications Required:Current NYS Physical Therapy License.Current BLS/CPR certification.Experience Preferences:Outpatient experience is necessary.Inpatient/acute/rehab experience is preferred.Notes:Shift & Schedule:Monday–Friday, 8:00 AM–4:30 PM (unpaid 30-minute meal break).Regular Saturday coverage (4 hours) on an intermittent basis (approximately every 4th or 5th week).o On weeks with Saturday coverage, 4 hours will be trimmed from another day in the preceding week.Location:Travel to other network locations only in extenuating circumstances.
