Registered DietitianRequired Qualifications:Registered Dietitian with current IL licensureExperience providing nutrition/dietetic services, preferably in a long-term care, skilled nursing, healthcare, or residential care environmentExperience performing nutritional assessments and developing nutrition care plansAbility to document assessments and care plans in an Electronic Medical Record (EMR)Knowledge of medically prescribed and liberalized dietsAbility to review menus, recipes, dietary operations, and food-service practicesKnowledge of HIPAA/privacy and confidentiality requirementsAbility to work onsiteMust provide current resume and proof of applicable IL licensureKey Responsibilities:Conduct nutrition assessments for residents and develop nutrition care plans upon admission.Review and revise resident care plans at least quarterly, after significant condition changes, or when requested.Evaluate medically prescribed diets and monitor residents' nutritional status.Document assessments, evaluations, monitoring, and care plans in residents' medical records.Review and modify menus using long-term-care dietary concepts, including liberalized diets.Observe meal preparation and service and conduct kitchen inspections.Recommend improvements to dietary and food-service operations.Assist in developing dietary policies, procedures, resident diet manuals, and recipes.Develop and provide quarterly in-service education to Facility personnel.Submit written reports following visits detailing findings, recommendations, progress notes, and progress against previous recommendations.Attend and assist with regulatory surveys when requested by Facility management.Coordinate scheduled visits with the Dietary Manager/Home Administrator and provide advance notice regarding planned absences.Perform other dietetic services requested by food-service management and Facility operations.Notes:
