The Registered Nurse will complete a report for each member in the format required by client, documenting the medical necessity for skilled services the member can access to meet their individualized needs. In-person assessments every 90 days and as needed. The position is field based, and travel is required to complete in-person assessments. Completes nursing referrals and places vendor calls Completes special projects as needed Completes documentation in the timeframe established Interprofessional collaboration internally and externally. Participates in multidisciplinary planning meetings to determine and identify appropriate services per policies and guidelines established by client’s contract
Duties:
Excellent communication skills including advanced abilities in explaining complex information and being able to educate patients and families.
Collaboration with members and members’ support systems
Attend mandatory meetings
Completes required annual trainings
Requirements:
Registered Nurse licensed to practice in the State of Arizona
Recent experience in caring for people with medical feeding tubes, including gastrostomy tubes.
Minimum (4-5) years of recent RN experience working as a Registered Nurse with individuals in home health, hospice, long-term care, or case management.
Additional training or certification in behavioral health, complex medical conditions, or nursing case management is preferred.
At least (2) years of experience working with individuals with disabilities.
Valid Arizona driver’s license
Arizona fingerprint clearance card required
Moderate to advanced experience with Microsoft Word, Google Suite, and Adobe
Preferred Skills
