We are seeking the support of contract nurses who can support the rising nursing caseloads that we are experiencing across the state.Description of Duties:The role involves conducting nursing assessments and collaborating with external agencies to ensure appropriate services are delivered. The assessments are used to authorize skilled nursing services for members.The successful candidate will conduct in-person visits statewide and have a core district that they serve, monitor health needs, assist with referrals, and advocate for members’ rights.The Registered Nurse will complete a report for each member in the format required by the client, documenting the medical necessity for skilled services the member can access to meet their individualized needs.Completes nursing referrals and places vendor callsComplete special projects as neededCompleting documentation in thetimeframe established by the client Interprofessional collaboration internally and externally.Participates in multidisciplinary planning meetings to determine and identify appropriate services per policies and guidelines established by the client’s contract with the client.Collaboration with members and members’ support systemsAttends mandatory meetings Completes required annual trainings.Experience and Expertise:Additional training or certification in behavioral health, complex medical conditions, or nursing case management is preferredExcellent communication skills, including advanced abilities in explaining complex information to members and members’ support systemsProficient with Google Office, Microsoft Office, and Adobe Acrobat, including instant message features, document uploading, and a willingness to learn new software used for documentation for the state.Requirements:Registered Nurse licensed to practice in AZ, active and in good standing without restrictions.Required SkillsMinimum (2) years of recent RN experience working as a Registered Nurse with individuals in home health, hospice, long-term care, or case management.License: RN in AZNotes:Hybrid8:30 am - 5:00 pm.In-person assessments every 90 days and as needed. The position is field-based, and travel is required to complete in-person assessments.
