We are looking for a compassionate Senior Social WorkerYour contribution:Outstanding Service: Provide the best assistance to veterans to improve their quality of lifeContinuous Improvement: Participate in facility meetings, training sessions, and the implementation of the client to achieve continuous improvement and efficiencyTeam Collaboration: Work with other professionals to use your expertise while improving your knowledge, skills, and abilitiesJob Duties:Plans, organizes and supervises the social services program of the client Ensures timely completion of assessments and documentation per federal and state regulations and facility policies and proceduresEnsures compliance with the PASRR (Preadmission Screening and Resident Review) process Supervises subordinate staff to ensure the goals and objectives of the work unit are metPlans, organizes, assigns, and reviews the work of the client staffReviews and evaluates job performance of staff through monthly 1 on 1 coaching sessions and annual performance appraisalsAddresses staff problems and recommends disciplinary actionsInterviews and selects staff Oversees clinically appropriate discharge planning to ensure it is safe and communicated effectively to all involved stakeholdersMonitors Medicare notices to ensure timely delivery to beneficiaries per federal lawCompletes consults for referrals to specialist at the Veterans Administration (VA)Works with residents, families, and staff to ensure that veterans/members are treated in accordance with Resident’s RightsAssists in projecting department budget working within budget parameters and monitors expendituresMaintains a personal caseload of assigned veterans/membersAttends and participates in various meetings and serves on committeesKnowledge, Skills & Abilities (KSAs):Knowledge of:CMS, and State regulatory standardsSocial Services programs and principles of social workGeriatric psychiatric issuesGeriatric disease processesSide effects of psychotropic medicationsPhysical and chemical restraintsCommunity resourcesGovernmental policies and programs that affect our clienteleLong-term care billing standardsMDS systemSkills in:Verbal and written communicationCounselingCase managementResident assessmentDeveloping care plansAnalytical skillsProblem solvingSupervision and managementAbility to:Establish and maintain interpersonal relationships at all organizational levelsUtilize the facility's IT program and maintain and record assessments and care plansMaintain the highest level of confidentiality
