Description:Mental Health Center – Mental Health SUD Program LeadBasic function:The Mental Health Center is a Certified Community Behavioral Health Clinic in the client. We operate several programs and clinics within the Mental Health Center, which includes Intensive Outpatient SUD program. SUD Program Lead is responsible for facilitating Co-Occurring Recovery program for adults, and/or SUD Youth Co-occurring Program facilitated by other LADC/SUD program lead provider. Each SUD lead will manage cases of clients individually, complete Comprehensive Evaluations, DAANES, and assure intake responsibilities that meet 245G and Clinic CCBHC standards are completed. SUD program includes SUD therapist as a part of the core team requirement.Examples of work performed by LADC: (Adult CORe and Youth CORe)- Enter client into MNITS for insurance verification for Direct Access- Complete preliminary risks assessments as needed- Schedule initial assessments as needed- Complete Comprehensive Assessment in allotted timeframe (60 days or 9 sessions)- Complete intake packet program policy and procedures, rules, guidelines, ROI, and signature pages- Complete DAANES intake process- Complete individual treatment planning (collaborate as needed)- Facilitate groups 4 times week/3 hours durations- Complete progress notes and documentations daily- Complete other assessments for MAT program intakes as needed- Meet with clients for 1 to 1 weekly- Complete all paperwork relevant for 245G audits: weekly treatment update, dx plan criteria, treatment received, screening tools- Be available to cover other groups as necessary to cover unexpected absences- Offer and facilitate family psycho-ed group sessions- Complete yearly required trainings for licensure and RC requirements: includes but not limited to CPR, NARCAN, HIV- Track trainings- Check Electronic Health Record inbox for SUD referrals- Meet for weekly group consultation w/SUD team- Meet weekly for individual consultation with Supervisor- Make referrals as needed following PHI regulatory for sending records- Assist with other clients at Mental Health Center- Complete peer review audits as required- Attend Care Coordination and present when it’s your time to present on client(s)- Scan completed documents into Electronic Health Record- Create curriculum for group including relapse prevention strategies- Assist with walk-ins as needed when in clinic- Connect with community partners and internally for program outreach and resourcing- Update treatment plans every 90 days and Comps yearly- Schedule family meeting w/ YCORe at least 1-2 times month and attend school as needed to complete comp assessments, etc.Notes:8:00am- 4:30pm or 9:30am-6:00pm
