Job Description


Job Responsibilities may include, but not limited to:

Facilitate the implementation of a life skills curriculum, titled Next Steps.
Utilize the Transition Coordinator Handbook in an effort to standardize the transition practices.
Evalute the five "Transition Consultations" to be completed by every student under the guidance of an assigned teacher.
Participate on a weekly basis in Administrative Release Committees (ARC) in each facility to confirm and reinforce the student's reentry plans.
Do the Follow Up Phone Calls to released youth at 1, 3, 6, 9, and 12 months.
The Transistion Coordinators will represent the client at many conferences and meetings.
The Transistion Coordinators will assist in the development of a partnership with client.
The Transition Coordinators will assist in the development of the individiualized Education Learning Plan for each youth upon entry at the facility.
Work with state, county and private agengies to provide re-entry servicers for youth transitioning back into the community.
Assist students with obtaining social security cards, state identification, driver’s license, birth certificates, and additional documentation.
Assist students with transitioning back to public schools
Assist students with employment, apprenticeship programs, and military service information

Skill Required / Desired
Prior working exp. utilizing strong communication skills (written and verbal). Required
Computer knowledge: Microsoft Word, Outlook, and Excel as well as PowerPoint. Required
Prior experience working as a customer service rep. Required 1 Years
Prior data entry working experience. Required 1 Years
Prior experience working in a Team environment. Required 1 Years
Prior exp. working w/at risk youth. Highly desired
Prior working experience in a community or State program. Highly desired
Prior experience working in a youth educational program. Highly desired

Notes:

Monday through Friday, 7:30am to 3:30pm
