The Transition Specialist for the client provides oversight to all transition-related practices in the client's juvenile correctional facilities.Job Responsibilities may include, but not limited to:Facilitate the implementation of a life skills curriculum, titled Next Steps.Utilize the Transition Coordinator Handbook in an effort to standardize the transition practices.Evaluate the five "Transition Consultations" to be completed by every student under the guidance of an assigned teacher.Participate on a weekly basis in Administrative Release Committees (ARC) in each facility to confirm and reinforce the student's reentry plans.Do the Follow Up Phone Calls to released youth at 1, 3, 6, 9, and 12 months.The Transition Coordinators will represent the client at many conferences and meetings.The Transition Coordinators will assist in the development of a partnership with the client.The Transition Coordinators will assist in the development of the individualized Education Learning Plan for each youth upon entry at the facility.Work with state, county and private agencies to provide re-entry servicers for youth transitioning back into the community.Assist students with obtaining social security cards, state identification, driver’s license, birth certificates, and additional documentation.Assist students with transitioning back to public schoolsAssist students with employment, apprenticeship programs, and military service informationSkill - Required / Desired - Amount of ExperiencePrior working exp. utilizing strong communication skills (written and verbal). - RequiredComputer knowledge: Microsoft Word, Outlook, and Excel as well as PowerPoint. - RequiredPrior experience working as a customer service rep. - Required - 1 YearsPrior data entry working experience. - Required - 1 YearsPrior experience working in a Team environment. - Required - 1 YearsPrior exp. working w/at risk youth. - Highly desiredPrior working experience in a community or State program. - Highly desiredPrior experience working in a youth educational program. - Highly desiredNotes:Position is on-site, Monday through Friday, 7:30am to 3:30pm. 37.5 hours a week.
