Description:

Duties:
Perform wound assessments, reassessments and management for all patients and per policies and procedures
Develops skin care programs to prevent skin breakdown of residents
Participate in the development and implementation of policies and procedures to manage residents with draining wounds, fistulae and/or tubes; wound prevention, wound care, ostomy care, and incontinence
Revise plan of care as indicated by the resident's response to treatment and evaluate overall plan for effectiveness
Assesses and reassesses pain
Utilize appropriate pain management techniques and educate residents and family regarding pain management
Serves as a resource to nursing and other facility staff by providing in-services on wound prevention, wound care, ostomy care, and incontinence
Communicates appropriately and clearly to nursing staff and Physicians
Provides in-services on wound prevention, wound care, ostomy care and Incontinence for the nursing staff as necessary
Communicate appropriately and clearly to nursing staff and physicians
Consults other healthcare professionals as appropriate to provide interdisciplinary approach to the resident's needs
Ensure that documentation meets current standards and practices
Attend training and continuing education classes in the areas of wound prevention, wound management and ostomies and stays current with changes, new advances/treatment in wound care

Requirements:
Current license to practice as a Registered Nurse or Licensed Practical Nurse in AZ
Certification as a Wound, Ostomy, and Continence Nurse (CWOCN or Certified Wound Care Nurse (CWCN)
Current CPR card that was obtained by physical assessment

Selective Preferences:
Experience in a skilled nursing facility/long term care environment

Preferred Skills
2 Step TB Test

Notes:
Schedule: The team is flexible on the schedule.
Options: 7am-3pm, 8am-4pm or 8am-5pm.
8hrs Monday- Friday- No weekends.
